Vision Trip Application

Below is the LGM vision trip application. You can fill out our application online or download it to your computer and scan back to Evan via email at evan@leannegoffministries.org. Please ensure you download all documents to complete your application. Please note that your liability release must be notarized and mailed to us prior to the trip’s start date. Your application will not be complete until all forms are received.

Upon submitting your application, a $300.00 deposit will be due.
Vision Trip Cost: $1,500.00 (Please Note: This payment does not include airfare or costs associated with visas or travel insurance)

You can pay for this trip using this specific link. Please write “Vision Trip” in the memo line of your payment. Please do not mention Cuba. If paying by check, please make checks payable to Leanne Goff Ministries.

Travel Insurance

In order to travel internationally with LGM, we require that you purchase travel insurance that covers missed or delayed flights, loss of baggage, and unforeseen accidents and circumstances that would require in country treatment or return to your country of origin, etc.

Below is a link to Global Benefits, LLC and agent, James Smith, whom we have used for several years. He is very knowledgable in what he does and will help you immensely. You can use your own insurance, this above is only a suggestion, but your policy MUST cover all of the above mentioned details and not just health insurance.
